Insurance Coverage
As acupuncture gains acceptance, more insurance companies have started
including acupuncture benefits for their customers. Although some policies
typically cover acupuncture only if the treatment is for anesthesia or pain
control, others may pay for most conditions treated. You can call your
insurancešs customer service center and inquire as to whether acupuncture is
covered. Let the agent know why you are seeking treatment [for example,
back pain, or headaches]. Dee Ann is a provider for several insurance
plans. Even though Dee Ann is a provider, coverage is not always
guaranteed. Each individual policy is different on coverage and benefits.

If your insurance carrier is listed above, we will submit your treatment
information and claims, provided acupuncture is a covered benefit on your
plan. Your insurance policy is an agreement between you and the carrier.
We can not make your insurance company pay for your treatments. You are
responsible for any unpaid charges, deductibles, copays and other applicable
coinsurance.

If you fax or mail to me, 5 business days
before your first appointment, the following information :
1. An enlarged copy of BOTH sides of your insurance card.
2. Your social security number.
3. The health condition you wish to treat.
4. Your name and phone number.
5. Your date of birth.
6. The policy holderšs social security number if other than yourself.
